As we continue to strengthen our Aged Care at Home services across Queensland, we are seeking an experienced and values-driven Clinical & Care Partnering Manager to join our team in North Queensland (based in Townsville or Mackay).

This is a key leadership role within our new Service Region model, focused on delivering high-quality, person-centred care and strong clinical governance across the region.

Your Role

As the Clinical & Care Partnering Manager, you will lead and oversee the delivery of clinical care and care management services in the community, ensuring safe, consistent and high-quality outcomes for clients.

Working closely with the Service Region Manager and multidisciplinary teams, you will provide clinical leadership, strengthen service integration, and support continuous improvement across the region.

Key Responsibilities

Clinical Leadership & Service Delivery

  • Provide operational leadership for the delivery of evidence-based clinical services and care management
  • Oversee practice in clinical assessment, care planning and ongoing reviews aligned to client goals and preferences
  • Support best practice clinical interventions including chronic disease management, wound care and palliative care
  • Promote a person-centred, coordinated approach to care across multidisciplinary teams

Quality, Risk & Compliance

  • Ensure services meet Aged Care Quality Standards, legislative requirements and organisational policies
  • Lead clinical risk management, including incident oversight, investigation and continuous improvement actions
  • Monitor clinical governance frameworks and implement strategies to enhance safety and quality of care
  • Conduct audits, reviews and reporting to support compliance and accreditation

People Leadership & Partnering

  • Provide professional guidance, mentoring and support to clinical, allied health and care management teams
  • Foster a culture of collaboration, accountability and continuous learning
  • Build strong relationships with internal stakeholders and external partners to enhance service coordination

Operational Oversight

  • Ensure accurate and compliant clinical documentation and reporting
  • Support service planning, performance monitoring and continuous improvement initiatives
  • Contribute to regional service strategies in partnership with the Service Region Manager

About You

You are a confident clinical leader with a strong commitment to person-centred care and quality outcomes. You bring a collaborative mindset and a passion for supporting teams to deliver exceptional care.

You will also have:

  • Current AHPRA registration as a Registered Nurse
  • Demonstrated leadership experience in community or aged care services
  • Strong understanding of Aged Care Quality Standards and regulatory requirements
  • Experience in clinical governance, risk management and quality improvement
  • Excellent communication and stakeholder engagement skills
